Medical Conditions that May Prevent you from Diving

Prior to diving all passengers must fill out a medical questionnaire. Certain Medical conditions whilst not always, may prevent you from taking part in scuba diving. The form shown below will give you an indication as to what medical conditions may prevent you from diving. By answering yes to any of the questions shown on the form does not necessarily mean you cannot dive, it does however mean that further assessment is required by a licensed dive doctor.
